The survey collects information from full-time and part-time certified teachers that includes but is not limited to: 

• the position vacated;
• the employee’s years of service in the position and in the district;
•  if the employee is taking a similar position in another district; and
•  the reason or reasons provided for leaving the district.

No personally identifiable information about respondents will be collected or stored as part of the survey.
